Revisions for DG Annotation Webform

2021-07-20: Liz edited form for general public usability: changed instruction language, added email field, edited Annotation Tips page, removed instructions to enter Last Name, First Name, and set the Annotation Type Form component to Private under Edit > Webform > Annotation Type > Display. This means that component will not display to users who are not logged in, but users who are logged in will still see it.
